Cunningham should be completely off. Outhaul should not be completely blown off though. Remember, sailing by the lee is all about flow across the sail. If you do not have a proper shape you will lose flow and will not go fast.
48 inches seems to be the standard today. Some guys go a bit longer but I wouldn't do it. If your extension is 48inches or shorter you should be just fine with a bit of practice.
Hell yes roll tacking is worth it. Rolling the boat through a tack does not make you gain speed, it helps you sustain the speed you had prior to a tack. Slamming the tiller hard over to tack acts as a brake and will slow you down every time.
